The above information will be used by Media Services to ensure the proper setup of your account. We appreciate your cooperation in completing this Service Schedule. As soon as it is submitted along with your Certificate of Insurance naming payroll company as Additional Insured, any applicable fully executed union agreements, chart of accounts and script, we will begin setting up your account. Our Workers’ Compensation department will contact you if they have additional questions about your project. Our Business Affairs department will send you a formal Payroll Services Agreement, if one has not been executed between the above referenced production company and the specific payroll company to be connected with your account. Reference is hereby made to the said Payroll Services Agreement. In the event of any disagreement between the Payroll Services Agreement and this Service Schedule, the Payroll Services Agreement shall control. The Payroll Services Agreement must be on file in order to complete your account set up. You may request a draft of the Payroll Services Agreement prior to completing this Service Schedule.
